ANTI-DEPRESSANT MEDICATION REVIEW FORM

Please only complete this if you have been asked to do so.  If you have been asked to complete this form, you are due to have your anti-depressant/anxiety medication reviewed.

This form will be reviewed, and we will then be able to re-issue this medication on your repeat prescription if appropriate.  Please allow 5 working days if possible.

You may be asked to make an appointment with a GP to discuss your answers further. 

If your mood has been stable for 6 months or longer, you may be able to reduce or stop treatment with advice from a GP.

Should anything change in your circumstances before your next medication review is due, please arrange a routine appointment to discuss your concerns.
