Please provide the practice with your most up to date mobile phone number.  We plan to use this to send you appointments reminders and provide you with health information, including sending results by text.   We need your consent to do this.  When you are next at the surgery please advise reception if you are happy for this to take place.  Please note, it is your responsibility to let the practice know if you change any of your contact details.

From 1 April 2019 the law changed and gabapentin and pregabalin are now classed as controlled drugs.  The timeframe for ordering your prescription has changed as have some rules on how the Practice can prescribe.  Please see the important information below.

1.  A maximum of 30 days' supply is allowed to be supplied on prescription, so your quantity on prescription may have been reduced.

2.  If you are taking gabapentin/pregabalin regularly as prescribed, you will need to request a prescription every 28-30 days.

3.  Prescriptions might not be issued more frequently than every 30 days.

4.  Lost prescriptions will not be replaced.
